What Is the Difference Between an Inlay and an Onlay?

Both are restorations made to fit a prepared area of a tooth. An inlay sits within the raised cusps of the chewing surface, while an onlay extends over one or more cusps when the damage involves a larger portion of the tooth.

The distinction is mostly about how much of the tooth needs to be repaired. Your dentist does not choose between them from the name alone; the remaining healthy tooth structure and the location of the damage determine how much coverage is appropriate.

When Is a Filling No Longer Enough?

A filling works well when the damaged area is limited and there is enough healthy tooth around it to support chewing forces. As the missing or weakened area becomes larger, simply placing a bigger filling can put more stress on the remaining walls of the tooth.

An inlay or onlay can provide a custom-shaped repair when a direct filling would be too large or when a weakened cusp needs protection. More coverage is not always better. The best repair protects the tooth while preserving as much healthy tooth structure as possible.

How Does an Onlay Compare With a Crown?

A crown covers the entire visible portion of the tooth above the gumline. An onlay only covers the areas that need reinforcement, which can preserve more natural tooth structure when the rest of the tooth remains strong enough.

A crown may still be the better choice for a tooth with extensive damage, large fractures, significant wear, or too little healthy structure to support a more limited restoration. The right decision depends on the tooth, not a rule that one type of restoration is always more conservative or durable.

What Happens During Treatment?

The damaged or decayed portion of the tooth is removed and the remaining tooth is shaped to receive the restoration. A digital scan or impression records the area so the inlay or onlay can be made to fit the tooth and bite.

Once the restoration is ready, it is bonded or cemented into place and the bite is checked. The finished tooth should feel comfortable when you close and chew. If the bite feels high or unusual after the numbness wears off, let the office know so it can be adjusted.

Can Inlays and Onlays Replace Old Fillings?

Yes, when an old filling is failing and the surrounding tooth needs a stronger or more precise repair. A worn filling does not automatically mean the entire tooth needs a crown, and an older metal filling does not need to be replaced simply because of its color.

How Long Do Inlays and Onlays Last?

Longevity varies with the material, the amount of tooth being restored, your bite, home care, diet, and whether you clench or grind. No restoration should be described as permanent, and the same onlay can perform very differently in a lightly loaded tooth than it does in a tooth exposed to heavy bite forces.

Regular dental exams give us a chance to check the margins, bite, and surrounding tooth. If you grind your teeth, a night guard may be worth considering to protect the restoration along with the rest of your smile. Changes are easier to manage when they are found before a restoration becomes loose or the tooth fractures.
