When Does a Tooth Need a Crown?

A crown may be recommended after a large fracture, extensive decay, repeated filling failure, root canal treatment, or significant wear. Crowns can also restore teeth that are badly misshapen or discolored when the tooth needs structural coverage as well as a cosmetic change.

The key question is whether the remaining tooth can support a more limited restoration. If a filling or inlay can provide a dependable repair, there is no benefit to removing more healthy tooth structure than necessary.

Crown, Filling, or Inlay and Onlay?

A filling repairs a smaller area of lost tooth structure. An inlay or onlay provides custom coverage when the damage is more extensive but the tooth does not need to be fully covered. A crown surrounds most or all of the visible portion of the tooth above the gumline.

The choice is based on the size and location of the defect, the strength of the remaining walls and cusps, and the forces the tooth must handle. A larger restoration is not automatically better simply because it covers more.

What Happens During Crown Treatment?

The tooth is shaped to create room for the crown while preserving the healthy structure that can remain. A digital scan or impression records the prepared tooth, neighboring teeth, and bite so the restoration can be designed accurately.

A temporary crown may be placed while the final restoration is being made. At the delivery visit, the dentist checks the fit, contacts, appearance, and bite before the crown is cemented. If the tooth feels high after the numbness wears off, let the office know so the bite can be adjusted.

Why Are Crowns Common After Root Canals?

Root canal treatment removes diseased tissue from inside the tooth, but it does not rebuild tooth structure that was already lost to decay, fracture, or previous fillings. Many back teeth need additional coverage afterward because they must withstand strong chewing forces.

A crown can protect the remaining tooth from splitting or breaking. Not every root-canal-treated tooth needs the same restoration, so the final plan depends on which tooth was treated and how much sound structure remains.

Crowns for Cracked or Broken Teeth

A crown may help when a crack or fracture weakens the outer structure but the tooth is still restorable. The dentist needs to determine how deep the damage extends before deciding whether coverage is enough.

Some cracks extend below the gumline or into the root and cannot be repaired reliably with a crown. If the nerve is damaged, root canal treatment may also be needed. If the tooth cannot be saved, extraction and replacement options can be discussed.

How Long Does a Crown Last?

A crown can last for many years, but it is not permanent. Decay can develop around the margin, the ceramic or other material can chip, the supporting tooth can fracture, and the bite can change over time.

Daily brushing and cleaning between the teeth are important because the crown depends on the health of the tooth and gum around it. If you grind or clench, a night guard may help protect the crown along with the rest of your teeth.

Dr. Ravi and his staff have been AMAZING!!! My husband Joe Sanchez needs implants because his teeth are not good. He wanted to get titanium implants so he can eat whatever he wants without much restriction. So in May '26 we decided to get them done at this location on 75th Avenue with Dr. Ravi. Dr. Ravi is an amazing dentist who TRULY cares for his patients! My husband currently has plastic implants until his gums heal completely for the permanent implants. This past Wednesday evening 9/16/26 upon "biting down" into a piece of bread, part of the plastic broke and that meant he could not eat much of anything. He texted Dr. Ravi (he gave Joe his personal number in case of any issues), and he responded the next morning and found out he was on vacation, out of the country. But that didn't stop Dr. Ravi from helping us. He told Joe to call the office so they can see him that same day (Thursday). Then Dr. Ravi called his office to give them a heads up so they can be ready to help Joe when he got there. He didn't have to do that but he did! In the beginning... The day before Joe was going in to start this procedure, Dr. Ravi called him (on a Sunday) just to give us peace of mind and to answer any questions we might have about what was going to happen the next day. THAT was impressive because he called on his day off and he seemed sincere. Of course I still had my doubts because of past experiences with dentists. Joe was asked to go back later the same week so they can make sure everything is looking good. But before he can go back in, Dr. Ravi called Joe in the evening to check on him. The office is only opened Mon-Thur. and Memorial weekend on Sunday, Joe lost a screw in the morning and the temp implant got loose, he texted Dr. Ravi (I had to convince him to as this was urgent), and he responded! He called Joe back and told him he was going to try to get someone to meet him at the office so they can help Joe. And he said, "Joe if I can't get a hold of anyone (since it's Memorial weekend), I will personally come to your home if you'd like, so I can help you. But if you can wait, one of my staff is going in on the holiday and we can do it then." That's what we did. What other dentist do you know that would be willing to go out of their way for you?! No one! There was another time that Joe needed to get in over the weekend and again, Dr. Ravi was there! There were also all the texts and calls that Dr. Ravi made to Joe just to see how he was feeling. He didn't have a staff do this, he did it himself. When I say, Dr. Ravi is amazing, I really mean it! I don't know any other dentist who would go this far for any patient. We are so very pleased with Dr. Ravi and his staff for truly caring and as they say, putting your money where your mouth is... My husband is almost ready for his permanent implants and we are so excited! We HIGHLY recommend Dr. Ravi and his staff because they genuinely care for their patients and go above and beyond so you can get the care that you need, and have the confidence in him and his team! I will do an update once Joe get's his permanents and maybe he'll let me add a pic. :)

I broke a temporary bridge. As usual Toni went above and beyond to squeeze me in the very next morning. She and Bernice worked hard to make me a new temporary, but it kept breaking. Finally, Toni thought outside the box and put my old one back on and repaired the broken part. I am a happy camper!
